Realms of Despair has over 30,000 active player files.

I don't know for certain the total number of unique individual people playing, but it is well over 1000 regular day-to-day players for certain.

With respect to player base vs. votes in, RoD's ratio is much more realistic than some of the other MUDs listed in the top ten.
